Employment Commitment About The Driver

本人受雇于George Shoe Efhiopia PLC，担任司机工作，除遵守公司的规章制度外，并愿意履行以下各项工作倏款，如有违背，愿承担责任并接受处分。

I am employed by George Shoe Ethiopia PLC as a driver.I hereby commit that, besides those regulations and rules of the company, I intend to be subject to the following working clauses, to aume liability and to be punished in case of any violation.一、本公司之车辆（大货车、人货车、小客车）均系公务车，未经批准不得作为私用或代人运货载人。如因此而发生事故者，司机应承担全部责任。经查实者，除负责赔偿公司损失外，予以辞退处分。1.No official vehicles of the company, including trucks, lorries and mini-buses shall be used for personal purpose or for conveying paengers and goods for other person without authorization.The driver shall fully aume the liabilities for any accident, and, if the violation has been confirmed, he/she shall be responsible for compensation for loes of the company and be dismied.二、司机对分配之车辆应随时进行维护保养并做好记录，任何时间均须保持清洁可用状态。经查实没有保养者，记大过一次处分。

2.The driver shall, in no time, maintain and service the vehicle distributed to him, and make records.The vehicle shall be clean and available at any time.The driver will be subject to a record of major demerit in case that he is confirmed to fail to maintain the vehicle.三、出车前应确实检查车容、水、机油、车灯及各种证号，并注意自己的服装仪容。行车中必须精神集中，专注驾驶，严格遵守交通规则。如闯红灯、违规停车、超速超载、酒后或无证驾车或私带违禁品，经公安、交警查处者，其罚款和后果由司机负担。

3.The driver shall surely check the vehicle appearance, water, engine oil, lamps, all certificate numbers and his dreing and look before departure.When driving, the driver shall, not only mentally concentrate and drive carefully, but also strictly observe the traffic regulations.The driver shall bear penalties and be liable for the consequences if he is investigated by the(traffic)police due to jaywalking, illegal parking, over speeding and overloading, driving under the influence, unlicensed driving and carrying contrabands.四、司机出车时必须填写派车单，经主管批准后方可出车。

4.The driver must fill out the Vehicle Dispatching Form before driving and depart with approval of the supervisor.五、按派车单所报备的行车路线和目的地行车，未经上级通知不得改变（特殊情况除外）。

5.The driver shall drive the vehicle towards the driving direction and destination routed in Vehicle Dispatching Form.The direction and destination shall not be altered without notification of supervisor unle otherwise in exceptional cases.六、在装卸物品时应协助搬运货物，确实检查并关好后车门。

6.The driver shall aist in carrying goods when handling the goods, and indeed check the rear door before closing it.七、载运公司废料垃圾时，检查垃圾倾倒情况，发现可用之物品应立即捡回并向公司领导报告处理。7.If the driver is carrying wastes of company, he shall check waste dumping, pick up the items which are found to be available and report to the leaders of the company.八、司机出车不得搭载未经核可的无关人员，经查实者记大过两次。

8.The driver is not permitted to carry any other unneceary persons that have not been approved yet.Violator who has been ascertained will be subject two records of major demerits.九、司机在外出车，应积极与公司保持联系。

9.The driver shall actively keep in touch with the company when he/she is dispatched.十、司机出车回厂应向公司值班保安及主管回报，并在派车单上注明返厂时间。

10.The driver, after returning to the factory, shall report to on-duty safeguard and supervisor, and clearly indicate the return time on the Vehicle Dispatching Form.十一、上班时间不经报备核准不得离开厂区，如全厂公休日则以轮值方式 留两人在厂值勤。应到而未到者，记大过两次处分；特别勤务不在此限。

11.During work hours, except for persons on special duty, no one is permitted to leave the factory area unle otherwise reported and approved.On legal holidays, two persons shall be on duty in the factory in turns.The absentee will be subject to two records of major demerits.十二、司机未出车时应在指定地点待命或进行车辆保养，不得随意离开工作岗位，未有临时出车不得提前进餐。

12.The driver shall be on standby at designated place or maintain the vehicle if he/she is not dispatched, and shall not have meals ahead of schedule unle he/she is temporarily dispatched.十三、车辆在厂内维修时，司机应主动协助维修，需厂外维修应事先填写《车辆外出维修申请单》，经批准后方可外修。

13.The driver shall actively aist in repairing when the vehicle is repaired in the factory.For the vehicles that need off-site repair, the driver shall, in advance, fill out Application Form for Off-Site Repair for approval.十四、司机借款必须附上相关凭证单据及派车单，如欠缺单据或未按规定登记里程，则不予核销费用。14.Expenses of the driver shall be enclosed with relevant vouchers and Vehicle Dispatching Form.These will not be written off due to absence of the vouchers or failure to register mileage as per rules.十五、洁身自好，不利用职务贪污受贿或做有损公司利益之事情，违者将公司有关规定处理，性质严重者，/ 3 将送交公安部门。

15.It is prohibited to use the position for corruption and bribery, or to damage interests of the company.Any violator will be punished in accordance with relevant regulations of the company, and may be handed over to public security authorities if serious consequences have been caused.十六、公司所有车辆之保养和安全检查由班长负责督导完成，如查实执行不力者，予以记大过一次。16.All company’s vehicles shall be maintained and checked for safety under responsible supervision of the squad leader.The confirmed weak enforcement will lead the squad leader to a record of major demerit.十七、车辆之年审、保险、养路费用和其它相关费用规费的缴纳概由司机提前申报，如延误不报，其罚款由司机负担。

17.The driver shall, in advance, report the payments for annual inspection of vehicle, insurance, road tolls and other relevant fees;and shall bear the penalties caused by delayed report.十八、认真执行《车辆管理办法》，服从上级工作安排。

18.The driver shall implement Measures on Vehicles Management in earnest, and shall abide by working arrangements of supervisors.受雇人签字：
