1□ NYIT Graduate Application Form /NYIT 研究生申请表

2□ Official transcript for four-year undergraduate studies /4 年本科成绩单(2)3□ Proof of Degree /大学学士学位证书(3)

4□ Official transcript for the prerequisiteand 3 core graduate courses /由中方院校开出的研究生预修课和三门核心课程的成绩单(成绩均须在B 以上)5□ GMAT score / GMAT 考试成绩报告

6□ TOEFL or IELTS score /托福或雅思考试成绩报告(4)

7□ Notarized affidavit of support/财务担保书-要盖公章

8□ Notarized bank statement /银行存款证明-要盖公章

9口Copy of paport/护照复印件

10-12口 Other application materials/其它相关申请材料

其它相关申请材料包括：推荐信（2封），简历（中英文各一份），户口复印件，照片4张（6个月内拍摄的2英寸x2英寸（51毫米x51毫米）正方形白色背景的彩色正面照）

Note – 注意:

(1)Each document requires to have the English translation-材料要有英文版，最好附上中文原件。

(2)If a student has not got the grade for the last semester and/or diploma，please indicate that it will be submitted at the arrival-学生若尚未取得第四年成绩或学位证书，要说明到 NYIT报到时能提交即可。

(3)Graduate of three-year college shall be admitted conditionally and required to take 30-credit bridge courses 三年大专可被有条件录取，再补30 学分课程。

(4)The student will be first enrolled in the English Language Institute if has no TEOFL orELT score-若没有托福或雅思成绩就要先申请英语语言学校(网上填写申请表)。
