财力证明表的内容

Certificate of Financial Responsibility 这个表格是在checklist里的'，intl学生一定要求的，财力证明表。

表上有这么一项要求：Unsigned forms will not be accepted.

但能签名的有两个地方

1.I have read the information the applicant provided. I believe all information is true and accurate, and the funds are available.

2.The information I have provided here is accurate and complete. I will not need additional financial suport from the xx University.

If any information changes before I enroll at the university, I will notify the xx U International Center immediately. I also understand that ma-ki-ng false or fraudulent statements within this Certificate of Financial Responsibility could result in disciplinary action.

我不知道该不该签。。。

还有这个表上要贴照片，需要这么早吗?我总感觉这张表就是个形式。
